Please read and follow the instructions below. All of these items must be completed as required, and submitted to the Director of Religious Education in order for enrollment to be complete. Please send all required items to:

1.) Baptismal Certificate. If your child is baptized, a copy of your child's baptismal certificate must be submitted for our records.

2.) Media Release. Click to download and complete the Media Release form. This must be submitted.

3.) Safeguard the Children Booklet. Click to download and read the Safeguard the Children booklet.

4.) Teaching Touching Safety Permission Slip. Click to download and complete the Teaching Touching Saftey Permission Slip form. If you do not wish for you child to participate in this course, you must fill out the Opt-Out Form for Teaching Touching Safety.

5.) Early Childhood Registrants. A payment of $125 is required for Early Childhood registration. A child is not considered enrolled until this payment is received. This payment is non-refundable. Please make checks payable to St. Monica Church.
